What to bring with You

• Current insurance card and photo ID
• Completed Registration Forms (OB patients must complete BOTH the patient registration and OB patient registration forms prior to their visit with the nurse)
• Completed Medical History Form
• A list of your current medications
• Any new medical/surgical conditions since your last visit.
